This death record has a few things that are interesting.

1. Her husband P.B. Ford (Peter Burrell Ford) was listed as her father by 
mistake.

2. The Medical Doctor did not really list a cause of death and forgot to sign 
the original document.

3. The Registrar forgot to sign and file the certificate back in 1911.
