Cindy was born in Salt Lake City, to Dott Mitchell and E. Forrest Cullimore on March 3, 1952. Raised in American Fork and Murray, Cindy graduated from Murray High School in 1970 and was married the following year to Steve Jacobsen, from 1971-1980. In 1977, Cindy gave birth to her only natural child, her daughter, Tracy Diane. Cindy started her career as a single working mother about three years later with Costco Wholesale where she was a key member of the office administration and an overall warehouse department "expert". Cindy loved and was loved by everyone she came into contact with, making success and perfection appear to be seemingly effortless tasks. She was loyally employed with Costco for 15 years. In 1999, Cindy found and married her soulmate, James Ray Allen of Cove and was employed with the family business, Allen Rock, shortly after moving to Cove. There she worked for about five years, until she was no longer physically able, due to serious health problems. Cindy and husband "Jim" raised oldest son Casey, daughter Jamie, and youngest son Caleb together through good times and bad, with immense efforts despite the struggles. Above everything else in her life, being a mother was most important to Cindy. She will forever be loved, missed and cherished. Survived by father Forrest and wife Billie, brother Mike and wife Carolyn, daughter Tracy, husband James, and son's Casey and Caleb and daughter Jamie. Preceded in death by her mother Dott, sister Joan, and brother, Hal. Funeral services will be held at 12:00 noon on Thursday, September 29 at the Cove Ward Chapel, 12832 North 1200 East, Cove, UT. Bird, Dorothy LaVon Dorothy LaVon Bird "Dancing with Dad Again" Dorothy LaVon Bird passed away September 27, 2005 in Salt Lake City, UT. Born March 23, 1935 in San Francisco, CA to Norval and Josephine Wallin Horrocks. Married Frank Bird May 11, 1953; marriage later solemnized in the Salt Lake LDS Temple, he preceded her in death on July 13, 2000. Dorothy was a lifetime member of the D.U.P. and was very active in the LDS Church, holding many positions. She wrote and directed many road shows and plays. She loved music, dancing, camping, and traveling to visit many LDS temples with her husband. She was a very caring person, always thinking of others and loved her family and friends dearly. Survived by Ralph (Shelly) Bird, daughter-in-law, Colleen Bird; Neal (Karen) Bird; 12 grandchildren; five great-grandchildren. Preceded in death by her son, Dave. Published in the Deseret News on 9/29/2005. Bohnet, Gerald Victor Gerald Victor Bohnet 1937 ~ 2005 Spanish Fork-Gerald Victor Bohnet, 68, died Sept. 19, 2005. He was born April 6, 1937 in Foremost, Alberta, Canada to Victor Emmanuel and Isabel Bohnet. Gerald married Sydna Rae Abbott on March 19, 1958 in the Salt Lake Temple. He is survived by his wife Sydna Rae, his five children Vicki (Samuelu) Matafeo, Toby (Malou) Bohnet, Darwin (Mandy) Bohnet, Kevin (Lana) Bohnet and "Little" Jerry Bohnet; 15 grandchildren and four great grandchildren. Other family members include James Bandeaux, Danielle Sika and Liz Requilman, their spouses and children. Together with Sydna they opened their home to dozens of foster children that called them Dad and Mom. He is also survived by sisters Donna Hague; June Fox, Jesse Ratcliffe, Nola Hertslet, Dianne Steed and brother Donald Bohnet. Gerald was preceded in death by his parents. Gerald grew up in Canada, received his first degree from Brigham Young University and taught in several high schools in Canada including Banff Composite High School, the family moved to Laie, Hawaii where he taught at BYU- Hawaii until his retirement three years ago. Gerald loved a good science fiction novel or movie, traveling (trains being his favorite), college sports, music, humor, gardening, walking "Shadow," genealogy (www.bohnetfamily.org) and all of his church callings. Bott, Kristine (Tolman) Kristine Tolman Bott Our Loving, Precious Wife, Mom and Grandma Kristine Tolman Bott was born May 5, 1953 and passed away September 26, 2005 due to the enormous challenges of inflammatory breast cancer. Kris met this trial with dignity and grace and never complained. Kris is the daughter of Don B and Hazel R. Tolman, twin sister to Kathy Reed and sister of Mike Tolman. Kris was raised and educated in Bountiful where she graduated from Bountiful High School with high honors. She later attended BYU. Kris waited for her best friend Robert while he served his mission in Canada. Shortly after his return they were married for time and eternity on June 2, 1972 in the Salt Lake LDS Temple. From that day forward Kris dedicated her life to her husband. She never wanted anything for herself but was always thinking of others. She was so unselfish. She learned to enjoy camping and going to the Greys River to fish and be with her family. Oh how we will miss the trips to the cabin in Star Valley and to Jackson and Cody! Until her illness Kris served faithfully in many organizations but her favorite was the primary. She served as primary president, in the young women presidency and in the relief society. Kris always supported Robert in his callings and served hundreds of Navajo tacos to his young men. Kris loved to cook and built a great family tradition of Sunday dinner every week. Even with her illness and as sick as she was she wanted the family around her Sundays and all of the time. Kris loved being a mother and faithfully raised five children in the gospel of Jesus Christ. She always taught and supported three sons who became Eagle Scouts, received duty to God awards, served full time missions, and married in the temple. She raised her two daughters to become worthy daughters of God and how to be wonderful wives and mothers. Kris raised some very special children who she dearly loves and is so proud of. Kris loved her special daughters-in-law and grandchildren. Her family was her life. May we all follow her example to be an eternal family. There is a huge hole in our hearts, but it will be filled by our love and memories of having the best, our beautiful Kris. Kris emulated the true love of Christ in her life and through her good works. She loved serving others and never said anything negative. She has been the perfect example in every way. Kris enjoyed working for the Boyer Company for 11 years. She truly loved her work and her coworkers. Thank you Boyer Company and especially Diane "for waiting". She always wanted to go back but never made it. Shantale was born in Salt Lake City, Utah, on October 3, 1985, to Cristy B. Garay and Todd Julio. Shantale was a beautiful, energetic, carefree and fun-loving person. Her greatest joy in life was her daughter, Felicia, horseback riding and spending time with family and friends. In her 19 years she touched so many lives with her loving and unselfish ways. For all that knew her, she will remain forever in our hearts! We will miss you! Burk, Susan Marie Susan Marie Burk 1965 ~ 2005 Susan Marie Burk, 40 of Orem, UT, beloved daughter, sister, aunt and friend returned to the loving arms of her Heavenly Father, Sept. 27, 2005 surrounded by family. She was born February 2, 1965 to Ralph Burk and Helen Burk in Salt Lake City, UT. She attended school in Alpine, American Fork and Orem. She was a very happy and loving person who made friends easily. She regularly attended Wasatch House (Wasatch Mental Health) where she had numerous friends. She was a very choice and special spirit of her Heavenly Father. Susan blessed the lives of so many and, in turn, has been blessed with so many close and dear friends and family members who cherish her and will always keep her close and dear to their hearts. Our lives have been blessed and our days enriched for every moment we were able to share with our beloved Susan. Carlson, Pauline Gardiner Pauline Gardiner Carlson "1915 ~ 2005" Our cherished mother, grandmother, great-grandmother, aunt, and friend passed away peacefully at home September 28, 2005, at the age of 89. She was born December 11, 1915, to Eli Arthur and Margaret Dunford Gardiner. Married Floyd Alma Carlson in the Salt Lake Temple January 16, 1942. She was in the first graduating class of South High School in 1933. A devout member of The Church of Jesus Christ of Latter-day Saints, she will always be remembered for her great strength, enduring to the end, love of work, love of reading, love of the scriptures and the Gospel, and especially for her love of family. She was preceded in death by her husband, four brothers, and one sister. She is survived by her brother, Carlos Gardiner, son Paul, daughters Carolyn Carlson, Linda (Antonius) Hoefnagel, and Ann (Roy) Myntti. She is also survived by her nine grandchildren Mark Hoefnagel, Marie Sherrod, David Myntti, Denise Myntti, Michelle Ballif, Jill Whitmore, Gina Myntti, Emma Carlson, and Sheila Myntti, and a great-granddaughter Kara Myntti. Curtis 1914 ~ 2005 On September 27, 2005, one of the Lord's noble and stalwart sons passed away at his home from the effects of cancer. Dale R. Curtis, husband, father, grandpa, great-grandpa, brother and friend will be deeply missed. Dad left this life as he lived it - nobly. How can words do justice to a life so well-lived and so well-loved? How does a family pay tribute to a person whose life exhibited the very essence of all that is virtuous, lovely, of good report or praiseworthy? We are comforted that he is now in the presence of our Savior and will be able to do what he has always loved doing, which is serving others. Truly of him it can be said, "well done, thou good and faithful servant." As a family, we have the certain knowledge that death is not the end and this period of separation is only temporary. Dale was born November 14, 1914, in Salt Lake City, Utah, the seventh of ten children of Alexander and Genevieve Raine Curtis. Following graduation in business from the University of Utah, he joined the FBI. He subsequently married Muriel Morris of Stillwater, Oklahoma, on November 10, 1941, in the Salt Lake Temple. Throughout World War II, Dale served as an FBI Special Agent in the Hawaiian Islands. At the conclusion of the war, he returned to Salt Lake City to embark on a long and successful business career including, with his father, the Curtis Coal Company. In addition to his beloved wife Muriel, Dale is survived by two sons D. Jay (Kathryn) and Alan M. (Kay), and two daughters Barbara Hansen (David) and Janet Call (Brooke). A daughter, Marcia Oliekan, preceded Dale in 1993. At the time of his passing, Dale and Muriel were blessed with 22 grandchildren and 36 great-grandchildren. Dale is also survived by two of his younger brothers, Lindsay R. (Marjorie) Curtis, and LeGrand R. (Patricia) Curtis. As a lifelong member of The Church of Jesus Christ of Latter-day Saints, Dale was always blessed with a strong and valiant testimony of the Gospel of Jesus Christ and believed in continuous Church service to the best of his ability. His service included Regional Representative of the Twelve, Netherlands Mission president, Young Men's General Board, Director of Temple Square and Visitor Centers, member of a bishopric, and Sealer in the Laie Hawaii and Jordan River Temples. Dad was a loving husband and devoted father. His greatest joy was being involved in the activities of his family. Whether at Huntsville for the 4th of July or vacations with each of his children and their families at Lake Powell, Steamboat Springs or Yellowstone Park, he always loved being with his children, grandchildren and great-grandchildren. Funeral services will be held Friday, September 30, 2005, at noon in the Murray 23rd Ward Chapel, 755 East Three Fountains Drive (5010 South). Born November 26, 1932 in Copperfield, Utah to William G. and Catherine Peterson. She grew up in Copperton, Utah and was a long time resident of Lovelock, Nevada. She is survived by her six children, Terri Kerlin, Cathy Lee (Harry), Joe (Valerie), Tom (Maryann), Kelly Gibson (Ronald), and Eric (Misty), grandchildren, and great-grandchildren. Also her sister, Sharon Heaps (Bill), Gilbert, AZ, and brother Gerald "Butch" Peterson, Columbia, SC, and sisters-in-law Dorothy, Copperton, UT, and MarJean, Murray, UT. Hill, Jack Jack Hill 1932 ~ 2005 FARMINGTON - On Monday, September 26, 2005, our father, grandfather and friend, Jack Hill, ended his courageous battle with pancreatic cancer. He fought this disease with such dignity and strength. Jack was born October 17, 1932 to Cora Pearl and Melvin Hill. He married Mary Dawson (later divorced). He is survived by his daughter Natalie (Corey) Houmand, and his son Lance (Ruth) Hill, five grandsons, four granddaughters and one great-grandson. He was preceded in death by his son Ryan. He is also survived by his brothers Dick and Joe Hill. Jack lived an amazing life. He attended Davis High School and then Utah State University. As an All-American half-back for the Aggies, he had great success. He still holds the all time scoring record. He left Utah State to serve a mission for the LDS church in Canada. When he returned he chose to continue playing football and was drafted in the NFL by the Baltimore Colts; however, he signed with the Saskatchewan Rough Riders. After playing in Canada, he was signed by the Denver Broncos, where he played until his career ended due to a knee injury. He was inducted into both the Canadian Football Hall of Fame, and the Utah Sports Hall of Fame. Jack's life included a career in the car business. He was the owner of Jack's Auto Sales. He also continued his passion for football as he coached both his sons and grandsons in Little League. You didn't want to be a referee making a call against Coach Hill's team!! Jack loved the game of golf. He was a master of the short game, and was currently perfecting "the swing of the future." He has been a member of Oakridge for over 30 years, and his "buds" from the club were family. Their love and devotion to him will never be forgotten. Holley, Joel Joel Holley 1966 ~ 2005 Joel Newell Holley passed away September 26, 2005, of Burkitt's lymphoma. Joel was born October 23, 1966 in Riverside CA, the son of Karl Newell and Jean Farrimond Holley. He graduated from Olympus High School and served in the Lansing, Michigan mission. His marriage to Gina Lofton Holley, gave him a daughter, the joy of his life, Cami J Holley and a treasured step-son, Keith Lofton. Joni Van Drunen, his attentive companion, and her daughters Chelsea and Amber became endeared family. In his youth, Joel spent countless adventurous hours exploring the beauty of Millcreek Canyon; he bonded intrinsically to its fishing, stream, terrain, rocks, trees, flowers and animals. As an adult, being property manager of Log Haven, was like a match made in heaven; his synergy with nature created a legacy that will endure at Log Haven. Joel's winning smile, twinkling eyes and sense of humor are his enduring hallmark. Even in final desperate circumstances, he expressed appreciation to his family, friends and medical team who supported and comforted him. He is survived by his daughter, Cami J (15) and step-son, Keith; his parents; siblings and their spouses, Todd (Kathy), John (Sarinda), Marc (Heather), Eric (SoonBok), Jill (Matt) Morgan, Bethe (Kyle) Price, and Scott; and 11 nieces and nephews Preceded in death by his grandparents, Don and Ada Leishman Holley, and Harold and Erma Clark Farrimond; also by Bethany Lofton and Christa Lofton. Jonathan had served in Panama and the Texas Houston East missions. He was currently an agriculture student at Utah State University. He loved ranching, horses, being outdoors and dancing with his sweetheart Shari. He was born August 4, 1955, in Ephraim to Keith Carlyle and Roxie Bradley Keisel. He married Tammy Smith, later divorced. Ron was a very polite and courteous person. He was a very talented artist and shared his artwork with many friends and family. He had a great love for his dogs and enjoyed exploring the outdoors. Lindgren, Lola (Pagnanelli) Lola Pagnanelli Lindgren "Yolanda" Lola "Yolanda" Pagnanelli Lindgren, age 86, passed away peacefully September 27, 2005 at home surrounded by her dear family and friends. She was born June 23, 1919 in Salt Lake City to Giacomo "Jack" and Isolina Ciccola Pagnanelli. Lola was from a proud Italian family and grew up in "Little Italy" on the west side of Salt Lake City. She developed her nurturing nature early in life caring for her beloved younger brother Jackie. This nurturing behavior continued through her marriage of 63 years to Frank Lindgren, her first born son Craig, her daughter Pamela, her grandsons Lance and Travis; and the gems of her twilight years her great- grandchildren; Alex and Samuel Vandongen. Lola's younger years were spent by her husband's side during World War II and traveling where ever the Air Force took them. When they finally landed back in Salt Lake City, Lola helped her husband operate coffee shops for more than 50 years. Frank and Lola became coffee shop icons in Salt Lake City. Their last endeavor was serving the men and women of the Salt Lake City Police and Fire Departments by operating "Pam's Code-7," in the SLC Public Safety Building. Lola's true love was children. and they loved her. She has influenced many lives by her loving relationships with the children around her. It was not uncommon to find the neighborhood children "hanging out" at grandma's house. She cherished every moment with all the children in her life, Alex, Sam, Toni, Britney, Heather, Alyssa, Sarah, Danny, Heather, Summer, Amber and all the others. She'll always be watching over you. Lola is survived by her siblings, Jack, Mary, Jo, Elinor, Ida, and Lou; her children Pamela, Max and Becky; her grandchildren Lance, Travis, Cristal and Nicki, and her beloved great- grandchildren Alex and Samuel. Tricia was born in St. George, Utah on August 6th, 1956 to Daniel Rex and Barbara Eager Dutton. She grew up in the St. George area where she attended elementary and middle schools. She met her sweetheart and soul mate Donald Michael Lloyd and was married to him on December 1st, 1973 and later sealed in the Salt Lake Temple on September 12th, 1975. Tricia graduated from Hurricane High School in May of 1975. All that Tricia ever wanted to be was a mother. She was blessed with four beautiful children and three grandchildren. She absolutely loves her children and grandchildren with a love that transcends the bonds of this earthly life. Tricia learned and loved the talent passed on from her grandma Eager to create craft projects in the realm of quilting, needlepoint, and especially crocheting. Many people that she met have been recipients of the gift of her creations. Tricia is survived by her husband, Don; her children: Angela, Michael (Marliese), and Jessica Ann Nielsen (Adam); her grandchildren: Caine, Gabriel, and Samuel Nielsen; her parents: Rex and Barbara Dutton of Grand Junction, Co. Madsen, 23, of Payson, Utah passed away on September 26, 2005 from injuries he sustained in an auto accident involving himself, his classmates, and an instructor from Utah State University. He will be sorely missed. Curt was born January 11, 1982 to Kenneth F. and Tamra Madsen. He was an active, smart, kind-hearted little boy, and loved the world around him. Curt had a great love for the outdoors, but a particular fondness for fishing, shooting, and trucks. Our family was never surprised, really, when Curt would call home and say he had made a new purchase. It was always a truck, truck accessory (toolbox), or a new firearm. Curt loved to collect guns and target shoot. He was not a hunter though, he didn't have the heart for that. Curt was a lover of agriculture, and found great joy in farming related activities. He raised pigs throughout his high school years, and in a meat judging competition he entered, he placed first in Utah, and third in the nation. His farming fever wasn't diminished one bit when he left for two years to serve a mission for The Church of Jesus Christ of Latter-day Saints in Texas. He got off the plane with a set of Texas Long Horns and probably had a gun in his suitcase. We didn't check. Beneath his tough guy exterior, was the kindest, most gentle hearted little man any of us ever knew. He'd do anything for a friend, had an uncanny ability to generate laughter among family and friends, played the piano well, had a beautiful tenor voice (when you could get him to sing), and had a deep love for his family. He left behind his father, mother, brothers, Wayne (Donna), Mark (Amy), and Keith (Margo), and two sisters Monica and Melanie. Curt has two nephews he dearly loves, Braden and Tyler, and a niece, Catherine, he never met. All who knew Curt will miss him dearly. Happy trails little buddy! Ann lived most of her life in California where she worked many years as a film editor for Paramount and in later years as an event planner for the Park Plaza Hotel. Born to Brigham Heber and Alice James Vincent November 22, 1907 in Salt Lake City. Married William LeRoy Nesbitt on June 6, 1928 in the Salt Lake Temple. He passed away March 26, 1994. Mother was active in the LDS Church participating in Primary and was the President of the Relief Society. She spent her life in serving her family and had many friends. Everyone who received her loaf of bread considered themselves as blessed. She is survived by a daughter, Darlene N. (Arvil) Armitstead and two sons, William R. (Suzanne) and Reed A. (Colleen) Nesbitt; 18 grandchildren; 68 great-grandchildren; and five great-great-grandchildren. A daughter, LaVon (Bonnie) preceded her in death. Olsen, Hazel (Wennergren) Hazel Wennergren Olsen 1908 ~ 2005 Hazel Wennergren Olsen died peacefully at home in Salt Lake City Sept. 24, 2005. Born in Logan, Utah June 17, 1908 to Oscar and Caroline Wennergren. She owned and operated a beauty salon in Logan for many years until her marriage to Ed T. Olsen in 1976 when she moved to Salt Lake City. Ed preceded her in death in 1990. She is survived by numerous nieces and nephews and an "adopted" family through Ed Olsen: daughter Marlene Guio, granddaughter Michelle King (Lyman) and great-grandchildren, Cameron and Sierra who loved and cared for her until her death. We have many wonderful memories of Grandma Hazel: teaching us to make baking powder biscuits, berry pies, fudge; enjoying her dinners of French toast on TV trays, New England boiled dinner, zucchini casserole, learning crocheting, going out for Hires burgers and root beer, bird watching, seasons changing, and planting gardens. You planted the seeds for a full harvest in our garden of life and gave us the daily sunshine of your spirit. We love you. Jean possessed the spirit and the work ethic of her pioneer heritage. She was known throughout the world for her hand crocheted afghans and for her homemade candy. Jean loved the mountains of Utah and Idaho, especially in the fall. Her children, grandchildren and great- grandchildren were her greatest joy. The lessons she taught her loved ones will endure for many generations. She is survived by her children Doralee (Jerry) Flannery, Sandy, Utah; Gordon (Terry) Player, Sun Valley, Idaho; Nancy Jean Coffman, Salt Lake City, Utah; and David (Vivian) Player, Boise, Idaho; 13 grandchildren and 20 great-grandchildren; sister Cleo Alvord, Boise, Idaho; and brother David Williams, Cedar City, Utah. Tamoua, devoted father and loving husband, son and brother, was called home to his Father in Heaven on September 24, 2005. Edwin was born June 15, 1970. He is the second son of Selita Valu and Paul Tamoua and grew up with brothers Paul Sime, Brian, James and sisters Darlene Toakase and Jamie-nia. Edwin was married for 13 years to Soana Malohi Tamoua and together they have been blessed with five beautiful children: Taina, 12, James, 10, Peata, 8, Christopher, 7, and Vaiokema, 6. Ed will forever be remembered for his big heart and generosity of spirit. He was protective of those he loved, and gave guidance to those who looked to him for wisdom and knowledge. He carried himself with a certain strength and ease that was rare among men. Ed had a talent for living life to the fullest. His hearty laughter was contagious and his sense of humor could quickly grab you and make you smile for days. Ed's outlook on life was always real and heartfelt. He reminded us that better things were always ahead and that if we really looked hard enough, the best things were always right in front of us. For those of us who were fortunate enough to know and love him, our lives have been blessed in countless ways. We have lost a father, husband, son, brother and friend in life, someone who brought us much joy and happiness beyond measure. We take comfort and hope in knowing that death is not an ending, but a beginning. Through his travels and life's adventures he made many friends, and he valued all of them. Steve was an avid outdoors man, he loved fishing and hunting. He had a deep passion for hoppin on "BEAU" and getting out on the open highway. He's done the road trip to Sturgis SD a few times with some great friends. Steve loved his family and friends and will be greatly missed by all. She could brighten a day with her usual "good morning" or a gentle kiss on your hand. Her powerful spirit was felt by everyone that knew her. Her physical challenges prevented her from walking, but with her sheer determination to discover this earth she compensated by rolling. She was a polite, cheerful and happy little girl and our "little princess". She loved her friends at school, teachers and her family. She loved music and would giggle at her favorite, Wheels on the Bus. Amberlee was a faithful member of The Church of Jesus Christ of Latter-day Saints and was always saying "Church". She loved to hear the kids sing and laugh. As she grew into a young woman, her problems seemed to multiply but her strength, grace and beauty shined only brighter. For the last eight years, part of her time was spent with the Michael and Mary Salls family, who lovingly helped care for her. They loved her and gave her two sisters, Jenny and Monique. They along with us marveled at her many accomplishments. She made gentle men out of her three strong brothers. She taught them many life lessons on compassion, service and love. Survivors include her parents Roger and Patti Ward; brothers, Tayler (Jessica), Lance (Ashly) and Preston; grandparents; Rachael Webb, Stanley and Emmalou Ward, and nephew, Saywer; many aunts, uncles and cousins who loved her dearly.
