She was born October 22, 1923 in Ogden, Utah. She married Bern Chadwick on her 18th birthday and had 3 beautiful daughters. She spent many years as a cafeteria manager on several military bases and fed our troops well. She was an active member of the LDS church. She spent much of her last few years enjoying the company of friends and family while playing cards and games. She is survived by her daughters Pat Fischer (Tennessee) and Linda Mead (Riverton, UT), sister Carrie Butcher (Farmington, UT), 8 grandchildren, and 28 great-grandchildren. She was preceded in death by her husband, Bern, daughter Sandy Petru, and several brothers and sisters. Funeral services for Hazel will be held Tuesday, November 22, 2005 at 6:30 pm in the Warenski Funeral Home Chapel, 1776 North 900 East, American Fork, Utah (801) 763-5000. Millstead, Matthew K. Matthew K. Millstead returned to his Heavenly Father November 19, 2005. Matt was born August 5, 1969 to Tamara McDaniel - American Fork, and Larry Millstead - Glendale, AZ. Matthew loved to fix cars and was the best there was at it. He worked for Geneva Rock for many years as a heavy equipment operator. He enjoyed the outdoors and looking for arrowheads. He had many friends that were all very important to him. Matt would do anything for anyone. He lived life to the fullest. He had many trials and struggles throughout his life, some he could not overcome. A number of these were his and several he would offload from others and take upon himself. Matthew is survived by his parents, Tammy McDaniel and Larry (Susan) Millstead; grandmother, Cleo Smith of American Fork; brothers, Brandon McDaniel of Lehi, Justin Millstead of Spanish Fork; sisters, Heidi Engle of Payson, Amie McDaniel of Pleasant Grove, Jessica Millstead of Las Vegas, NV, Jaclyn Marvin of Orem; Lois and Leland Hamton who he considered his Godparents; and many extended family members. He will be truly missed and loved. We would like to thank all of the staff at American Fork and Utah Valley Hospitals for all their love and care that was given. Page, Walter Gwynne 1915-2005 Gwynne was born August 28, 1915 in Lark, Utah, the son of English immigrants, William Oliver and Alice Morris Page. He passed away peacefully at his home of natural causes on November 19, 2005 at age 90. Gwynne married Ivy Cardwell, the love of his life, on December 9, 1935 in the Salt Lake Temple and began their life together on their Bluffdale farm, raising chickens, fox, mink, horses, and shetland ponies. He was an avid gardener, flowers being his favorite. He was an active member of the Church of Jesus Christ of Latter-day Saints, serving in many callings. With his wife Ivy they served in the Ohio, Cleveland Mission. He remained a faithful home teacher his entire life. Dad was a very loving, kind, generous husband, father, grandfather and neighbor; a true gentleman. Gwynne is survived by his wife, Ivy, and their 5 children, LeRoy (Darlene),Alpine, Jay (Peggy), Bluffdale, Nola (Chuck) Hobbs, American Fork, Wilma (Jerry) Wilkinson, Cumming, GA, Carolyn, (Mike) Bergstrom, West Jordan, 27 grandchildren, 64 great-grandchildren, two brothers, Morris (Jane), Bob, and one sister, Mildred Collett. Preceded in death by his parents, sister, Wilma Fowkes, 4 grandchildren, Holly Dawn Woodward, Sherie Lee Woodward, Vonnie Page, and Wendy Wilkinson. Whited, Larry Dee April 20, 1971-November 20, 2005 Our beloved Larry Dee Whited, 34, passed away early Sunday afternoon. Larry was born in Provo, Utah, son of C. Robert Whited and Rae Woolley. Larry grew up in Orem, the seventh of eight children. He was an avid reader and ran track for Timpview High School, graduating in 1989. He attended Weber State University. Larry was a hard worker and had a quick smile. He stood up for those who were down on their luck. He faced many difficult challenges in life and felt that his greatest accomplishments were his sons, Swayde and Payton. Larry will be greatly missed. He is survived by his former wife, Heather Goltry Whited; children, Swayde Spencer and Payton Robert; parents. C. Robert (Nancy) Whited, Rae Woolley Willoughby; siblings, Leslie (David) Whited-Vance, Carl (Carol) Whited, Steven (Brenda) Whited, John (Diane) Whited, Alan (Lisa) Whited, Kelly (Chris) Jones; stepbrother, Michael (Jocelyn) Nelson. He is preceded in death by his brother, Roger Whited, step-father Jesse Willoughby, and step-brother Terry Willoughby. Funeral services will be held Wednesday, November 23, at the Orem 1st Ward Chapel, 750 W. Center. Woodhouse, Hershel Beloved father, grandfather, and brother, Hershel (Hersh) Woodhouse of Payson, Utah, died on November 20. 2005 at the age of 90, after a long battle with cancer, with his son by his side. He joined his beloved wife and daughter today in a grand reunion of love and happiness. Hershel was preceded in death by his mother and father, Ethel Farthingham and John Thomas Woodhouse, daughter Sharon Woodhouse, and brothers Howard and Emil Woodhouse. Hershel was born February 22, 1915 in Lehi, Utah. He married Blanche Cravens on October 8, 1938. He served in WW II in the Navy. He worked at the Tintic Standard Mine in Eureka. Then being called by the war manpower commission he drilled wells for Geneva Steel, until the mid 40's. He then worked with his father in his well drilling business. After his father's passing he continued the business until retiring in 1991. Dad left his mark on the world, as many people will remember him when they turn on a tap! He loved the outdoors, fishing, hunting, going on trips with his son and being with his grandchildren. Hershel was a member of the American Legion and a member of the Church of Jesus Christ of Latter Day Saints. He is survived by his son: Michael (Shauna) of Payson. His greatest joy his 3 grandchildren Chelsey, Sara and Sharon. A brother Ralph of Payson. Services will be Wednesday November 23, 2005, at 11 :00 a.m., at the Walker Mortuary, 587 South 100 West, Payson, where friends may call one hour prior to the service. Hershel will be interred beside his wife in the Payson City Cemetery with military rites accorded by the American Legion District #4. Special thanks to Alpine Hospice team for all their help.
