No Patient was Seen twice on Thursday prior to his death and was Suffering very much from Effects of indigestion, did not See him on night of his death until after he died not being [able] to reach his home in time to See him alive. He lived about 30 minutes after he became ill, and judging from symptoms described I concluded ["I" struck] that death was caused by Acute Indigestion.
