Forward, died yesterday afternoon at 4:30 o'clock at the residence of her daughter, Mrs. George E. Creekmur, 3706 Colonial Avenue, after a long illness. Besides her daughter, she is survived by two sons, William W. and John W. Forward; a sister, Mrs. S.J. Burton, of New York City and three grandchildren. Funeral services will be conducted tomorrow afternoon at 3 o'clock.
