The Rev. Canon Bernard Kim Forman was born November 9, 1933 in Lodge Grass,
Montana to Percy and Lona Forman.

He trained as a Navy pilot in Pensacola, FL and was discharged in 1957.

In December 1957 he married Mary Travis.   Mary died in 2011.

Later in life he earned a Master's in Theology at Dallas Theological Seminary
and was ordained a priest in the Episcopal Church, Diocese of Olympia.

He was preceded in death by his parents, siblings and wife.

He is survived by his children:  Rev. John Forman (Jennifer), Betsy Serdar
(David) and Susan Blakely (Neal); and childhood friend Joanne Frizzell.
