Floyd was born September 8, 1920 to Carl and Gladys Levin in Walla Walla, WA.

He was preceded in death by his wife of 53 years Phyllis.

He graduated from the University of Washington in 1942, earning his MD from
George Washington University in 1946 and did his internship at Harborview,
Seattle, WA.

He is survived by his brother Elmer; step-daughters Sally (Werner) Kiepe and
Sharon Sievers.
