“To the Honorable Charles Dunn, presiding judge of the District Court and for 
the County of Grant, and Territory of Wisconsin:

I, Thomas Stephens an alien born free white person, of the age of thirty-three 
years, and upwards, do hereby in conformity with the first condition specified 
in the first section of the act of Congress entitled, “An act to establish an 
uniform rule of naturalization, and to repeal the acts heretofore passed on 
that subject,” approved the 14th day of April, 1802; declare and make known to 
the said District Court now sitting, in and for the county and Territory 
aforesaid, that my true and proper name is Thomas Stephens, that I was born in 
the County of Cornwall and in the Kingdom of Great Britain on the 10th of 
April, 1815 and that I am about thirty-three years of age; that I belonged to 
the English nation, and owed allegiance to the king of Great Britain; that I 
migrated from the port of Patstoe in the said Kingdom of Great Britain on the 
10th of April, 1840, and landed at the City of Buffalo, in the state of New 
York, in the United States of America on the 15th day of August, 1840, that I 
have ever since my arrival remained within the limits, and under the 
jurisdiction of the United States, etc…”
