D. one thousand nine hundred and seven personally appeared before me, a Notary Public within and for the county and State aforesaid, Samuel L. Daywitt, who, being duly sworn according to law, declares that he is 69 years of age, and a resident of Springfield, county of Greene, State of Missouri; and that he is the identical person who was enrolled at Boscobel Wisconsin under the name of Samuel L. Daywitt, on the 14th day of August, 1862 as a private, in Company "B" 33rd Regiment, Wisconsin Infantry in the service of the United States, in the Civil war, and was honorably discharged at Vicksburgh Miss, on the 9th day of August 1865. That he was not employed in the military or naval service of the United States otherwise than as stated above. That his personal description at enlistment was as follows: Height, five feet ten inches; complexion, dark; color of eyes, black; color of hair, black; that his occupation was farmer; that he was born November 10, 1837 at Carroll County Indiana. That his several places of residence since leaving the service have been as follows: Boscobel, Wis. then to Windham, Minn. then to Pittsburg, Kas, then to Ash Grove Mo., then Taney Co Mo. then Springfield, Mo. That he is a pensioner. # 931194. That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the act of February 6, 1907. That she is the widow of Samuel L. Daywitt Co. B 33 Wisconsin Inft. and who was a pensioner of the United States by certificate No. 931,194, on the roll of the Pension Agency at Topeka, Kan. ; that the last payment of his pension was made to the 4th day of Feb., 1910; that he died on the 28th day of March, 1910. That she was married under the name of Susan Vier to said pensioner at in Taney Co., Mo. on the 9th day of July, 1899 by A. J. Craig J.P. that there was no legal barrier to the marriage; that she had been previously married; that the soldier had been previously married. She was married first to Jesse Vier, who died in Russell Co., Ky. and he was married to _[ blank]__ That her post-office address is Springfield R.F.D. #5, County of Greene, State of Missouri Attest: (1) G. D. Chandler age 49 years, a resident of Springfield, in the county of Greene and State of Missouri, whose post office address is Springfield, Mo.; well known to be reputable and entitled to credit, and who, being duly sworn, declare in relation to aforesaid case as follows: We were well acquainted with Samuel L. Dewitt and know Samuel L. Dewitt and Samuel L. Dawitt is one and the [same] person from our acquaintance 18 years and 15 years respectfully.
