The document is a sworn affidavit dated April 29, 1902, from the Notary Public, Henry Pitts of Mineral Point, who swears that: "I examined the family Bible of the late THOMAS W. MITCHELL. I find the same was printed in the year 1872 by the American Bible Society. It was purchased by THOS. W. MITCHELL in 1873. I find the following entries in the spaces for family records: THOMAS WARD MITCHELL born July 10" 1843 Mrs. FANNY MITCHELL born April 4" 1843 William A. Mitchell born June 9" 1873 NETTIE G. MITCHELL born March 4" 1877 Benj. Y. Mitchell born October 9" 1879 Elsworth Thos. Mitchell was born July 4" 1881 Lee L. Mitchell was born March 28" 1886 Fanny E. Mitchell was born September 22" 1887 Tillie J. Mitchell was born March 18" 1889 Ida E. Mitchell was born October 5" 1890. Died March 22" 1901 Mary A. was born May 12" 1892. "On a separate page, I find this entry: Louisa J. Mitchell born June 4" 1854. Died July 14" 1896. "From the appearance of the writing I should judge that the first entries were made soon after the purchase of the Bible, all the succeeding entries were made soon after the events occured. There is no sign of any erasures having been made on any of the pages. From my acquaintance with handwriting of the late THOMAS W. MITCHELL, I should judge that all the entries made in the Bible were made by THOMAS W. MITCHELL himself. Henry Pitts, Notary Public, Iowa Co. Wis."
