Stemple & Forman Funeral Home
Jeremy Scott Mayle, 21, of Rt. #1, Philippi, died Monday, April 17, at his 
home, following a five month illness. 
He was born May 14, 1984, at Philippi, a son of John Edward and Lora Jane 
(Collins) Mayle, who survive at Rt. #1, Philippi. 
Also surviving are his paternal grandmother, Carolyn (Adams) Mayle, 
Louisville, Oh.; his paternal great grandmother, Mable (Croston) Adams, 
Philippi; aunts, Ava Klonowski, Louisville, Irene Bhaskarla, Hagerstown, Md., 
Reann Mayle, Sheila Collins and Gayle Miller, all of Philippi; uncles, Roy 
Mayle, Massillon, Oh., Jeff Collins, Philippi; and ten cousins. 
He was also preceded in death by his paternal grandfather, Johnnie Mayle; by 
his maternal grandparents, Howard Hayward and Mary Edna (Croston) Collins and 
by an uncle Terry Collins. 
Mr. Mayle was a truck mechanic and was a former employee of Benson and Sons, 
Trucking Co. He loved motorcycles and hunting and was Protestant by faith. 

(Funeral services will be conducted from the Stemple & Forman Funeral Home, 
385 So. Main Street, Philippi at 2:00 P.M. on Friday, April 21. The Rev. 
Marvin "Jack" Mayle will officiate and interment will follow in the Adams 
Section of the Welch Cemetery. 
Friends may call at the funeral home from 2:00 until 9:00 P.M. on Thursday and 
from 9:00 until 2:00 P.M., the service hour, on Friday.
