ACT OF FEBRUARY 6, 1907.


DECLARATION FOR PENSION.


The Pension Certificate SHOULD NOT Be Forwarded With the Application.

On this 29 day of April A. D., one thousand nine hundred and 1907 personally appeared before me, a Notary Public within and for the county and State aforesaid, Madison H. Koontz, who, being
duly sworn according to law, declares that he is 63 YEARS OF AGE, and resident of Busch Po county
of Roger Mills, Ty. of Oklahoma; and that he is the identical person who ENROLLED at Buffalo
West. Va. under the name of Madison H. Koontz, on the 3 day of December, 1861, as a Private, in Co.
D, 8th W. Va. Vol. Inft., afterwards was mounted and called the 7. West. Va. Vol. Cavalry in the
service of the United States, in the Civil War, and was HONORABLY DISCHARGED at Wheeling, West.
Va., about the 9 day of July, 1865.  That he also served - - -.  I was a veteran.  Reenlisted in
the same Co. and Regt.  Was a Corporal when discharged.

That he has not been employed in the military or naval service of the United States otherwise than
as stated above.  That his personal description at enlistment was as follows:  Height, five  feet 8 inches; complexion, dark; color of eyes, brown; color of hair, dark; that his occupation was
farming; that he was born Sept. 18, 1843, at Buffalo West Va.

That his several places of residence since leaving the service have been as follows:  Wise Co.
Texas, Grady I. T., El Reno Okla. Colony, Texola Okla. and Busch, Okla.

That he is - - - a pensioner by Certificate No. 667049, at $8 per month.  That he has - - -
heretofore applied for pension, Claim No. - - -, under - - -.

That he makes this declaration for the purpose of being placed on the pension roll of the United
States under the provisions of the act of February 6, 1907, and any amendments thereof.

He hereby appoints, with full power of substitution, MILO B. STEVENS & CO., of Washington, D.C.,
their successors or legal representatives, his true and lawful attorneys to prosecute his claim
under said law; and agrees that they shall be paid such fee or compensation as may be declared
allowable by law, ruling or decision.

That his post-office address is Busch, county of Roger Mills, Ty. of Oklahoma.

Also personally appeared C. D. Cowden (Cowder?), residing in Elk City, Okla Ty. and Dan Apple,
residing in Elk City, Okla Ty., persons whom I certify to be respectable and entitled to credit,
and who, being by me duly sworn, say that they were present and saw Madison H. Koontz, the
claimant, sign his name (or make his mark) to the foregoing declaration; that they have every
reason to believe, from the appearance of the claimant and their acquaintance with him of 15 years
and 1 years, respectively, that he is the identical person he represents himself to be, and that
they have no interest in the prosecution of this claim.

claim.
